02 · Scope
What we accept referrals for
Consultations and direct diagnostic orders. Testing is performed in our Clearwater office, so your patient is not sent to a third location.
General neurology consultation
Adult outpatient
- Headache and migraine — new, changed, or refractory to first-line management.
- Dizziness and balance — chronic or recurrent presentations, including unsteadiness and recurrent falls.
- Memory and cognitive concerns — reported by the patient or by a family member.
- Other general neurology — call (727) 210-7699 if you are not sure a presentation is in scope. We would rather answer the question than have a patient sent to the wrong place.
Direct diagnostic orders
No consultation required first
- EEG — routine outpatient electroencephalogram, recorded in our office.
- Transcranial Doppler (TCD) — ultrasound assessment of intracranial arterial blood flow.
- Cognitive testing — structured assessment of memory, attention, language, problem solving and processing speed, giving a measurable baseline.
Do not use this website for a medical emergency. If you are having a medical emergency, call 911 or go to the nearest emergency department. Symptoms of a stroke — sudden weakness or numbness, trouble speaking, or a sudden severe headache — are an emergency. This form and our phone line are not monitored around the clock.

A referral you send is your reputation as well as ours. This is the sequence on our side.
We contact the patient
Our team calls the patient on the number you send to schedule. If they reach us first, they can call (727) 210-7699 directly.
Seen within a week
Most new patients are seen within one week. Call to check current availability before you promise a date.
In person or telemedicine
Visits are available at our Clearwater office or via telemedicine. In-office testing and the initial neurologic examination are done here.
History and examination first
The visit begins with history and examination. Any in-office testing is arranged from there rather than ordered before the patient is seen.
04 · The closed loop
What comes back to you
A referral is only useful if the information returns. Our intent is that the referring office receives a consult note after the visit, together with the results of any testing we perform.
- Consult note. Turnaround time and delivery method are not published here yet. Call (727) 210-7699 and we will tell you exactly how notes will reach your office and confirm the fax or address to send them to.
- Test results. EEG, TCD and cognitive testing are performed in our office, so the report comes from the same practice that saw your patient.
- Clinical questions. Call (727) 210-7699 about a patient you have referred and ask for the clinician who saw them.
05 · Referral form
One-page referral form
A printable form is being prepared. Until it is posted, fax a referral on your own form or send a chart note. Please include the following.
- Patient name and date of birth
- A callback number the patient answers, and a second number if you have one
- Reason for referral, or the specific study you are ordering
- Relevant notes, current medication list, and reports for any prior imaging or neurodiagnostic studies
- Referring provider name, practice, phone and fax, so the note has somewhere to go
We are not publishing insurance or benefits information. Ask your patient to call (727) 210-7699 and our team will check their benefits with them directly.

The office is open monday to friday, 8:30am to 4:00pm. It is on N McMullen Booth Rd at Enterprise Rd, roughly one mile south of Mease Countryside Hospital, with free surface parking at ground level.
