Who provides your care
ASK Health Brain & Body Wellness is a comprehensive neurology and wellness practice at 2454 N McMullen Booth Rd, Suite 606, Clearwater, FL 33759, seeing patients in person or via telemedicine. You can read about the clinicians who provide care on our providers page.
Clinical services are provided by an independently owned medical practice.
What this website can and cannot do for you
Our appointment request form is a way to ask us to call you back. It is not a secure channel for medical information, and it is not monitored around the clock.
Please do not use this website to:
- report a new or worsening symptom;
- ask for test results;
- request a prescription or a refill; or
- send anything urgent.
