This page explains what happens to information when you use this website — what the appointment request form collects, what our web server records, and the things this site deliberately does not do.
What this policy covers
This is a policy about this website. It covers the appointment request form and the ordinary technical records that any web server keeps.
It is not our Notice of Privacy Practices. That is a separate document about your medical record and your rights under HIPAA — see our Notice of Privacy Practices page.
The short version
- This website uses no third-party analytics, no advertising pixels, and no session recording.
- It does not set tracking cookies, so there is no cookie banner.
- The only information you send us is what you type into the appointment request form.
- That request is emailed to the practice. This website does not keep a copy of it.
What you send us
The appointment request form
If you send an appointment request, we receive your first and last name, a phone number, an email address if you choose to give one, whether you would rather be reached by phone or by email, whether you are a patient, a family member or caregiver, or a referring provider, which area of care your request is about, whether you would like to be seen in person or by telemedicine, the times of day that suit you, and your acknowledgement of the notice printed above the form.
The form does not ask for, and will not accept, a description of your symptoms, a date of birth, an insurance member number, a Social Security number, or a file upload. Please do not include medical details, test results, or a description of your symptoms. Share your name and the best way to reach you, and a member of our team will call you to talk about your care.
Ordinary server records
Like almost every website, the computers that serve these pages keep short technical records of the requests they handle. Those records can include the internet address (IP address) a request came from, the page that was asked for, the date and time, and the type of browser and device in use. They are used to keep the site working, to protect it from abuse, and to limit how many times the same address can submit the form in a short period. They are not used to build a profile of you, and they are not combined with your appointment request.
What this website does not do
- No Google Analytics, Google Tag Manager, or any other third-party analytics.
- No advertising or social media pixels — nothing from Meta, Google Ads, TikTok, LinkedIn or X.
- No session recording, heat maps, or scroll tracking.
- No third-party chat widget or chatbot.
- No embedded map, video, or social content. The map on our contact page is drawn, not embedded, and only opens a map service if you choose to tap the link.
- Fonts, images and scripts are served from this site itself, not from an outside network.
This was a deliberate choice. The address of a page you read on a medical website can say something about your health, and we did not want that information leaving with anyone else.
Cookies
This website does not set cookies for advertising, analytics, or tracking, and it does not store identifiers in your browser to recognize you on a later visit.
What happens to your appointment request
When you submit the form, it travels to our server over an encrypted connection. The server checks it, formats it into an email, and sends it to the practice. Your request is not saved to a database on this website, and it does not sit waiting in an outside form dashboard.
The message is read by the members of our team who schedule appointments, so that they can call you back.
We do not sell, rent, or trade anything you send us. We do not share it with anyone beyond the practice and the service that delivers the message to our inbox.
How long it is kept
This website does not keep a copy of your request once it has been delivered. How long the practice keeps the email itself is set by the practice's own records policy. If you would like to know how long your request is kept, or you would like it deleted, call us at (727) 210-7699 and we will help.
Your choices
You never have to use the form. You can call (727) 210-7699 instead and speak to someone. If you have already sent a request, you can call us and ask us to correct or delete the contact details you gave.
Links to other websites
A few pages link out — to a map service so you can get directions, for example, or to a government website. Once you follow one of those links you are on someone else's website, and their privacy policy applies rather than ours.
Security
Information you send through this website travels over an encrypted connection. No website can promise perfect security, which is another reason to keep medical details out of the form and let us call you instead.
